Unfortunately everybody can follow you, without your permission for that. Each private session is active as long as you`re active on Spotify, if you`re not active for 6 hours the private session automatically is turning off.

One thing I should note though, if your family knows your user and can search you on Spotify, they may be able to see any playlists and your top/recently played artists on your account page. I recommend setting playlists to private and having all social aspects off for extra measure.
